An Evidence Based Debate
Safety and Efficiency of Nerve Blocks vs Other
Postop Pain Relief Regimens

10th October 2012

The Droitwich Knee Clinic hosted a very interesting scientific meeting, as part of its scientific clinical and audit meeting program. The theme on this occasion was postoperative pain control after knee surgery. It involved a fascinating debate addressing an unresolved question regarding the best, safest and most efficient pain control regime after knee replacement.

The Droitwich based Knee Foundation, which runs the scientific program, educating doctors, nurses and physiotherapists, pitched two eminent speakers against one another. Dr Barrie Fischer, an experienced consultant anaesthetist, who has recently retired from the Knee Clinic, but still works part time at the Alexandra Hospital in Redditch and lectures world wide on the topic, talked about the safety and efficacy of nerve blocks, with clinical evidence from his work and publications as well as scientific literature.

On the other side of the debate was the Knee Clinic's own Mr Kirti Moholkar who has been championing the enhanced recovery program, without nerve blocks, both at the Royal Orthopaedic Hospital and at the Knee Clinic, for early recovery and rapid discharge of patients.

As expected, sparks were flying in the discussion afterwards. This was moderated by Angus Strover, who had founded the Droitwich Knee Clinic back in 1988. The discussion included surgeons, anaesthetists, anaesthetic technicians and nurses.

The conclusion was that both techniques are valid as long as the team work the system around them and support by trained staff was available. Both techniques allow rapid mobilisation and safe discharge home within 3 to 5 days of the surgery.
